3. Analysis of Refinery Incident Database:

The 1984 Bhopal disaster has transformed industry's approach to prevention of major incidents in chemical plants that may disastrously affect the public. Chemical process safety has received attention across the globe after the Bhopal accident. In the U.S., OSHA PSM and EPA RMP have come into effect and are intended to minimize the risks to workers and the public. With an increased emphasis on safety in the processing industry, there are reports of incidents and causalities. In order to investigate the recent frequency of accidents, we examined reports of accidents in refineries all over the world between years 2000-2009. We relied on restricted-access databases, newspaper, magazines and company websites to gather information about the nature of the accidents, the severity, the processing unit, and possible causes for failure.
